Initial Comments:
An offsite follow-up survey conducted on December 27, 2021, January 3, January 14, and January 28, 2022, found that Reliant Home Health Agency, Inc. had corrected the following deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a recertification survey completed on August 30, 2021.

Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a re-licensure survey completed on August 31, 2021.
